Information you provide to a Health Stand Nutrition Consulting Inc. contact via an email message, fax or telephone call specific to your information request, inquiry and services information and/or booking an appointment, is deemed to be provided by you voluntarily with your consent. We will use the information you submit to answer your specific inquiry, request or book your appointment.

When you receive health and nutritional services from Health Stand Nutrition Consulting Inc., some personal information needs to be shared with your nutrition consultant and the staff of Health Stand Nutrition Consulting Inc. so that the right care, treatment and service can be provided.
We collect personal information from you such as name, home address, email address and telephone number(s), credit card information and health benefit information to:
- Open and update client files
- Invoice clients, process credit card payments or collect unpaid accounts
- Process claims for payment or reimbursement
- Send reminders and informational material to clients
Personal information may be disclosed to third party health benefit providers and insurance companies where you have submitted a claim for reimbursement or payment or have asked Health Stand Nutrition Consulting Inc. to submit the claim on your behalf.
We collect personal health information from you such as your nutrition, exercise and health history and your family health history, your physical condition, and any prior health services treatment you may have received for the purposes of providing you with nutritional and health services treatment and care.
The personal information that you are asked to give is collected, used and disclosed in accordance with the provisions of the Alberta Personal Information Protection Act (PIPA) and the Federal Personal Information Protection and Electronic Documents Act (PIPEDA).
