A Real-Life Example of How Low Iron Can Masquerade as Burnout

When 48-year-old Cassidy* who was seeing me for overall health and sports performance said, “I just don’t seem to have the same power and endurance I use to have when I’m working out. Whether it’s yoga or lifting weights, I just can’t seem to keep up like I used to. Is this normal aging?” Remembering that she had told me at a previous session that her perimenopause regularly resulted in long periods of heavy menstrual bleeding and that she also suffered from frequent headaches, I asked whether her iron levels had been tested recently.  

“Oh my gosh, I had low iron when I was pregnant, and this feels exactly the same!”  She was flabbergasted that she hadn’t recognized the signals coming from her own body. “I even have iron pills at home! I’m going to start taking them again.” I suggested she wait until she had the tests – after all, there can be different reasons for her symptoms, and we wouldn’t want to treat something incorrectly, I explained.  

She was tested, and it did turn out to be an iron deficiency, so we spent a session discussing all the ways she could tweak her diet to ensure that lots of iron-rich foods were included and covering ways to optimize absorption. After a few months of supplementing, she was able to stop the pills and maintain her iron levels through food alone. 

Today, six months later, Cassidy is back on her game and reported she was finally able to meet and exceed her exercise goals – doing 10 unassisted pull-ups in record time. I feel like a new woman,” she enthused. “Who knew one tiny little element like iron could make such a difference!”   

Iron deficiency in Canada is more common than many people realize

According to a 2023 Canadian Health Measures Survey iron deficiency anemia affects:

  • 10% of the general population
  • 29% of non-pregnant people aged 19–50 are iron-deficient

Iron also plays a key role in DNA synthesis and repair, as well as supporting the immune system. When iron is low, you may experience: 

  • Increased infections such as recurrent colds

    During pregnancy, iron requirements increase significantly—from the standard 18 mg per day to 27 mg daily—to support fetal growth and maternal health.

    Babies are born with iron stores sufficient for about four months. After this period, iron-rich foods become essential, which is why many baby cereals are fortified with iron.

  • Complete Blood Count (CBC) 
    This general blood test measures the number, size, and type of blood cells, including hemoglobin (Hgb)—the iron-rich protein responsible for carrying oxygen throughout the body. Low hemoglobin often indicates anemia. 
  • Serum Iron 
    Serum iron measures the amount of iron circulating in your blood. Most of this iron is bound to transferrin, a transport protein, but some may be free (unbound). Both low and high serum iron levels can be problematic—too much iron can lead to hemochromatosis, a dangerous iron overload condition. That’s why accurate iron testing is essential before starting supplements. 
  • Transferrin 
    Transferrin is the main protein that transports iron in the bloodstream. When your body’s iron levels drop, it produces more transferrin to capture and deliver as much iron as possible to cells that need it. 
  • Total Iron Binding Capacity (TIBC) 
    TIBC measures how many iron-binding sites are available on transferrin. A high TIBC indicates your body has many available sites but not enough iron to fill them, often signaling iron deficiency.

How Danika Found an Everyday Way to Boost Her Iron

Danika* had been diligently working to build her iron stores without taking a supplement as they resulted in stomach pain and nausea, and she didn’t want to add another medication to treat the side effects. The next time we met, Danika told me that she made the lemon water, cooled it in the fridge, added ice, and brought it to work all summer to sip on throughout the day. “The ladies in the office love it too!” She shared, “It’s such an easy way to get both iron and hydration and doesn’t upset my stomach like the pills did. And this winter, I’m going to have warm lemon iron water instead of tea. I can’t wait for the weather to turn cold,” she laughed.   

Considering Iron Supplements? Here’s What You Need to Know:

You may begin to feel more energetic after a few weeks, but it can take several months to fully replenish low iron stores, depending on the severity of the deficiency. Therefore, it’s important to find a supplement you tolerate well, as you’ll need to take it for an extended period.
